Enter the 10-digit mobile number registered with your Patient ID

Patient ID Information

  • Format: DSH + Year + Auto Increment (e.g., DSH202400001, DSH202400002)
  • Validity: Patient ID is valid for 15 days from your last visit date
  • Auto-Expiry: Patient ID automatically expires after 15 days of inactivity
  • Renewal: Your Patient ID is automatically renewed when you visit the hospital
  • New Registration: If your Patient ID expires, you need to register as a new patient
